North Node by Sign

I am assuming you list the North first, then the South. Is this correct? For example, if the node chart says I am Leo/Aquarius, then Leo is my north node and Aquarius is my south node.
Another question I have, can one ever have their birth sign be the same as their MC? I am assuming not, but I don’t know.
Also, how do I determine my houses?
